Auto-fill the unchanging top section once: type your Address, NMCS/Clinic/DEA numbers + expiration dates, pharmacist & tech names and license numbers, then click “Save top section as defaults.” Those will pre-fill every future week automatically.
Everything is remembered automatically. Anything you type or check is saved in this browser the moment you change it, and reloads next time you open this file. The Date of Review and the footer Date are set to today automatically each time you open it.
All checkboxes default to “Yes.” Click any box to mark it; click the marked box again to clear it. Each row is Yes / No / N/A.
Expiration warnings appear at the top (and highlight the field) when any NMCS, Clinic, or DEA registration is expired or within 30 days of expiring.
Saving the PDF: click “Save as PDF / Print,” then choose “Save as PDF” (or “Microsoft Print to PDF”) as the printer. The file name is pre-filled as OTP Weekly Pharmacist Site Review - YYYY-MM-DD using today’s date. Pick your folder and save. Sign the printed copy by hand.
